1. This form may take you 15-20 mins to complete.

2. Only 1 submission is allowed per applicant. Password will be given for applicant to re-access before submission.

3. Applicants are advised to verify all data entries before submission.

4. Applicants who are currently not in Singapore national schools, you are required to apply for a registration number at MOE Customer Service Centre before submitting your application to Nanyang Girls' High School.

5. The application will be closed on 6 July 2009, 5pm. Incomplete or late submission of application will not be processed.

6. Applicants are advised to have the following documents ready before proceeding to the next page:
   - your identity document such as Birth Certificate, Foreign Student Pass or MOE registration receipt.
   - your school results in primary 5 and 6.
   - your other academic achievements.
   - your CCA records and other achievements.

7. Applicants are required to post to the school within 5 working days upon online submission (confirmed), a complete set of the photocopied documents listed in point 6 stamped 'Certified True Copy' and a recent passport-size photo to:
